Healthcare Transformation Program
Swedish Hospital leads the Chicago North Side Collaborative, a Healthcare Transformation Collaborative.
The Chicago North Side Collaborative was awarded by the Illinois Department of Healthcare and Family Services (HFS) as a promising model for “person-centered, integrated, equitable, and dramatic change in the delivery of healthcare at the community level.”
As an innovative collaboration with Federally Qualified Health Centers (FQHCs) our Healthcare Transformation Program aims to eliminate barriers to specialty care and address social determinants of health for under-resourced communities.
Who is Eligible?
Patients of our referring Federally Qualified Healthcare Centers (FQHC)
Swedish patients without a PCP with either:
- Medicaid
- No insurance
- Private insurance and the inability to pay out of pocket costs
Dr. Maria Olga Cardenas - Endocrinology
Dr. Cardenas is board-certified in internal medicine and endocrinology, diabetes and metabolism. Dr. Cardenas speaks English and Spanish.
Dr. Joseen A. Bryant - Sports Medicine
Dr. Bryant is board-certified in sports medicine and family medicine. Dr. Bryant speaks English.
Dr. Carmen Patricia Macias - Pulmonology
Dr. Macias is board-certified in critical care medicine, internal medicine, pulmonary disease and sleep medicine. Dr. Macias speaks English and Spanish.
Dr. Amrita Kushal - Cardiology
Dr. Kushal is board-certified in cardiology. Dr. Kushal speaks English, Hindi and Spanish.
Dr. Charu Gupta - Cardiology
Dr. Gupta is board-certified in cardiology and advanced heart failure. Dr. Gupta speaks English.
Leah Fahey, RD, CDCES - Certified Diabetes Care and Education Specialist
Leah is a certified diabetes care and education specialist. She conducts individual consultations for any nutrition related concern. Leah speaks English.
