Scheduling Your Personalized Visit
Family Connects Chicago services are offered through in-home or virtual/phone visits to Chicago families who deliver at Swedish Hospital (at this time, families who live outside of Chicago are not eligible). A Family Connects representative will call you within the first few weeks after your hospital discharge to schedule this visit.
