Post-Acute Services

If your care team determines you are not yet ready to return home or could use extra assistance after discharge, there are several post-acute services available. Below is a comparison of these options to better understand the services offered by each.

Our units are located with Swedish Hospital, which offers 50+ academic-level medical specialties, including Emergency Department physicians. Swedish Hospital is certified by the Health Facilities Accreditation Program (HFAP) as a Primary Stroke Center.


Guideway Care – Care Guide Services

    At Swedish Hospital we believe your care shouldn’t stop when you leave the hospital. We know many patients and their families still need assistance following discharge. That’s why Swedish Hospital has partnered with Guideway Care to work with patients who have complex and/or chronic health conditions such heart failure, chronic pulmonary disease and stroke.

    If you have been identified as a patient who would qualify for and benefit from this service, you will receive a phone call from a Care Guide within 48 hours following your hospitalization. You do not need to do anything to be enrolled in this program and there is no charge for the service. Your hospital caregivers will start the process on your behalf.

    Your Swedish Hospital Care Guide will work with you and your caregivers to help with scheduling follow-up appointments, transportation, or provide information about services within the community.


    Swedish Hospital Inpatient Acute Rehab

      Physician Visits

      Patients are seen daily by a physiatrist. Other consultants may see patient as needed.  

      Nursing

      Certified nurses specializing in rehabilitation nursing are present 24 hours a day.  

      Length of Stay

      A patient's length of stay depends on the patient's medical conditions and averages 14 to 24 days, with a focus on home discharge.  

      Intensity of Program

      Therapy at least three hours a day, five days a week by qualified therapists: physical therapy, occupational therapy, speech-language pathology and recreational therapy.  

      Function of Program

      Patient and family education is integrated into daily care and encouraged to support successful discharge.  

    Swedish Hospital Extended Care Facility 

      Physician Visits

      Patients are seen several times a week by their primary care provider. Other consultants may see patient as needed.  

      Nursing

      Nursing care provided 24 hours a day. 

      Length of Stay

      A patient's length of stay depends on the patient's medical conditions and averages 14 to 24 days, with a focus on home discharge.  

      Intensity of Program

      Therapy at least one to two hours a day, five days a week, by qualified therapists and therapy assistants: physical therapy, occupational therapy and speech-language pathology. 

      Function of Program

      Patient and family education is integrated into daily care and encouraged to support successful discharge.
