Free Diabetes Education

You may qualify for free nutrition and diabetes education sessions.

The staff at NDC provides education on: 

  • Healthy eating 
  • Exercise 
  • Blood glucose monitoring 
  • Weight management 
  • And more 

Patients are eligible for the program if they have:

  • Any diagnosis needing nutrition or diabetes education
  • AND are uninsured OR have insurance that does not cover nutrition education for the specified diagnoses.

Program includes:

  • One initial and a minimum of two follow-up sessions with a Registered Dietitian or a Certified Diabetes Care and Education Specialist
  • Complementary enrollment in the Fundamental Fitness Program*
  • Video or audio interpretation services as needed
  • Information on food access resources
  • $50 grocery gift card after completion of third visit
  • Transportation vouchers to cover the cost of public transportation to their three appointments
  • $15 gift card after completion of two visits

For patients with prediabetes or diabetes:

  • Hemoglobin A1c analysis and tracking (Please bring your HbA1c results from your last doctor’s appointment).
  • Blood glucose monitor, test strips, and lancets to self-monitor blood glucose levels

Various other options are available for community members who have Medicare or commercial insurances.
