How to Schedule a COVID-19 Vaccine Using NorthShoreConnect

STEP ONE: Select the calendar icon (“Visits”)


STEP TWO: Select “Schedule an Appointment.”


STEP THREE: Select “Tell Us Why You’re Coming In” and select “COVID Vaccine”


STEP FOUR: Select the organization with whom you are a patient


STEP FIVE: Complete the questionnaire and schedule (if appointments are available)


Parents of patients 12-17 years old need to sign up for Family Access using their own NorthShoreConnect account:

  • Log on to your own NorthShoreConnect account
  • Under the Health menu click “Share My Record”
  • Under “Medical Tools," choose “Family Access (Proxy) Settings”
  • Click on the blue link that corresponds to the type of access you’re seeking
  • Complete and submit the request form
