Swedish Covenant Hospital's mobile medical HouseCalls program enables homebound elders to receive ongoing medical care at home.
Our nurse practitioners work in partnership with primary care physicians to provide comprehensive medical care to individuals who are no longer able to get to the doctor's office.
In Illinois, advanced practice nurses are registered nurses with a minimum of a master's degree in Nursing and advanced clinical training. They are certified by national professional organizations and licensed by the state. Working with collaborating physicians, they diagnose and manage many acute and chronic illnesses.
Nurse Practitioners employ advanced diagnostic skills and professional judgment to initiate and coordinate care for patients. They order diagnostic tests, prescribe medications and use therapeutic measures to treat illness and improve the health status of those in their care.
