Our nurse practitioners work in partnership with primary care physicians to provide comprehensive medical care to individuals who are unable to get to the doctor's office. In addition, the nurse practitioners collaborate with the program's medical director on a regular basis.
The HouseCalls program provides a wide range of services including but not limited to:
- Medical home visits by licensed, certified nurse practitioners
- A detailed health history
- Physical examination
- Specimen collection for lab analysis
- Home monitoring technologies like pulse osimetry and electrocardiograms (EKG)
- Wound care management
- Patient/Family education and support
- Follow-up care
- Referrals to other community resources
- Telephone access to on-call clinician 24 hours a day
- Collaboration between the nurse practitioners and the primary care physicians as well as the program's medical director
- Medication adjustments
The mobile medical HouseCalls program is the only hospital-based program of it's kind on Chicago's north side. The nurse practitioners serve as a bridge between the homebound patient and the physician by reporting significant changes or findings on a regular basis.
