What is a mobile medical program?
Mobile medical programs are programs that offer ongoing, routine medical care provided by a licensed practitioner who visits you in your home. Care is similar to the care you would receive in the physician's office.
Who is eligible for ongoing medical care at home?
Adult individuals who are unable to leave the home to access care in the doctor's office due to physical or psychological limitations or the need for costly special transportation are eligible for mobile medical care through the SCH HouseCalls program.
Will these visits be covered by insurance?
The HouseCalls program provides a range of services that are billable under Medicare, Medicaid and many private insurance plans. Self pay services are also possible.
How do I sign up for the HouseCalls program?
- If you do not have a primary care physician, please call the HouseCalls program and we will assist you.
- If you do have a primary care physician, please speak to the doctor about your needs. Your physician can then call the program with a referral for you.
What are the boundaries for the SCH HouseCalls program?
The HouseCalls program serves the needs of homebound individuals who reside in the area bordered by Lake Michigan to the East, Cumberland Ave. to the West (8400 W), North Ave. to the South (1600 N) and Dempster to the North (8800 N)
What should I do if I have a medical emergency while enrolled in the SCH HouseCalls program?
Although the HouseCalls program has a nurse on-call after hours it is not an emergency service. If you have a medical emergency, we recommend that you call 911 immediately.
