The Swedish Covenant Hospital Home Health Care department is under the Division of Nursing at Swedish Covenant Hospital. Our main office is located off campus at 2965 W. Peterson Avenue, Chicago, IL, 60659.
SCH Home Health Care's office hours are 8:30 a.m. to 5:00 p.m. daily, Monday through Friday. Our 24-hour number is 773-275-9700.
SCH Home Health Care has a liaison nurse on site in the hospital Monday through Friday during the day. The liaison can be reached at 773-878-8200, ext 5922.
For problems or concerns after hours, please call our 24-hour number, 773-275-9700. We have a nurse available by telephone seven days a week, 24 hours a day to assist with your call.

SCH HHC sees patients who live in the area bordered by Lake Michigan to the east and Cumberland Ave. (8400 W) to the west. Our southern boundary is bordered by North Ave. (1600 N) and our northern boundary is bordered by Dempster St. in Evanston (8800 N).
